Mechanical failures occur in all facets of life. In this case, a forklift’s fork was extended as it was being driven. Suddenly the fork dropped and killed an employee. An ARCCA mechanical engineer inspected the forklift and determined that the slipper blocks failed, causing the fork to drop. He redesigned and tested an alternate slipper block demonstrating that it would have prevented the fork from dropping.
