Many dangers exist in mining and major excavation projects. In one case, ARCCA was asked to determine if a site fatality involving a falling earthmover bucket happened because of operator error, mechanical failure or an inadequate design. The ARCCA expert inspected the earthmover and discovered that, in this instance, it was a mechanical problem, as a broken hose caused the bucket restraint to fail.
